Complex Needs Supported Housing Officer

We are a dedicated team supporting adults with complex needs, including those experiencing homelessness, mental health challenges, and social vulnerabilities. Our mission is to provide high-quality, person-centred support through coordinated, multi-agency approaches. Role Overview: We are seeking a committed and organised Complex Needs Support Officer to join our office-based team. This role involves working directly with individuals to assess needs, coordinate support, and maintain accurate records, ensuring that service users receive holistic, timely, and effective assistance. Key Responsibilities:Conduct assessments and develop person-centred support plans for adults with complex needs.Provide guidance and support to service users, including welfare, housing, and community resources.Monitor progress and liaise with internal teams and external agencies to ensure coordinated care.Maintain detailed and accurate records in line with data protection and organisational policies.Support safeguarding and risk management processes.Attend team meetings, case reviews, and contribute to service improvement initiatives.Person Specification:Experience working with adults with complex needs, including mental health, homelessness, or social vulnerabilities.Strong organisational and record-keeping skills.Ability to work collaboratively in a multi-agency environment.Knowledge of safeguarding, housing, and welfare systems.Excellent communication skills and a compassionate, professional ..... full job details .....
