Are you an experienced Complex Care Manager looking for a role where you can take real ownership of community-based care packages?

We are looking for a Complex Care Manager to oversee the delivery of specialist care packages across the West Midlands and Shropshire, supporting both adults and children with a range of complex clinical needs.

You''ll take responsibility for setting up new packages, maintaining existing services and working closely with clients, families, clinicians and commissioners to ensure consistently high standards of care.

The role As Complex Care Manager, you''ll oversee community-based packages supporting adults and children with needs including tracheostomy care, ventilation, spinal injuries, acquired brain injuries and bowel care.

Your responsibilities will include:

• Setting up and coordinating new complex care packages
  
  
• Overseeing existing packages and maintaining high standards of care and compliance
  
  
• Carrying out regular client visits across the West Midlands and Shropshire
  
  
• Completing and signing off person-centred care plans
  
  
• Working closely with clinicians and MDTs to review complex care packages
  
  
• Building relationships with ICBs, local authorities and other commissioners
  
  
• Preparing and submitting reports to commissioners
  
  
• Leading, supporting and supervising care teams
  
  
• Completing staff supervisions, appraisals and team meetings
  
  
• Supporting rota management and ensuring packages remain appropriately staffed
  
  
• Supporting safeguarding, incident management and service improvement
  
  
• Participating in the management on-call rota
  
  

What we''re looking for To be considered for this Complex Care Manager position, you should have:

• Strong experience within community or domiciliary care
  
  
• Previous management or senior leadership experience within complex care
  
  
• Hands-on knowledge of complex clinical packages, such as ventilation, tracheostomy, spinal injury, acquired brain injury or bowel care
  
  
• Experience supporting complex care packages for adults and/or children
  
  
• Strong knowledge of CQC requirements and person-centred care
  
  
• Confidence working with families, clinicians, commissioners and wider MDTs
  
  
• Strong leadership and problem-solving skills
  
  
• Full UK driving licence and access to your own vehicle
  
  
• Flexibility to travel across the West Midlands and Shropshire when required
