You will play a vital role in promoting independence, dignity and wellbeing while delivering person-centred support tailored to each individual.Key ResponsibilitiesProvide high-quality, person-centred support to individuals with learning disabilities, autism and mental health needsSupport clients with daily living activities, including personal care, meal preparation and maintaining their home environmentEncourage independence, choice and community inclusionSupport individuals to attend appointments, activities and social opportunitiesFollow individual care plans and risk assessments at all timesMaintain accurate and timely documentationWork flexibly across weekdays, evenings and weekendsCommunicate effectively with colleagues, families and other professionalsEssential RequirementsMinimum 6 months'' experience in the care sectorCaring, compassionate and empathetic natureReliable, hardworking and committed to delivering high-quality careAbility to work both independently and as part of a teamGood communication and interpersonal skillsCommitment to safeguarding and ..... full job details .....
