We''re offering a 12-month fixed term opportunity to become a Community Support Worker at Amplius.Working closely with customers, colleagues and partner agencies, you''ll provide person-centred support that helps people facing homelessness move towards greater independence, wellbeing and long-term housing stability.Salary: £28,040.47 per yearContract: 12-month fixed term contractYour week: 37 hours - Monday to Sunday between the hours of 8am - 10pm on a rolling rota.Location: Gainsborough HRS, Lincolnshire Snapshot of your roleProvide housing-related support tailored to individual needs, helping customers maintain and increase their independence.Act as a key worker, developing and reviewing support plans and risk assessments with customers.Support customers to manage their tenancy, including rent payments, utilities, reporting repairs and maintaining their home.Build positive relationships with customers, encouraging social inclusion, wellbeing and engagement within the local community.Work with statutory and voluntary agencies to coordinate and deliver effective support programmes.Maintain accurate records, reports and support documentation, monitoring customer progress and outcomes.Promote safeguarding, health and safety, equality and customer involvement while managing risk appropriately within ..... full job details .....
