Community Outreach Support Worker - Mental Health £25,877.80 per annum Peterborough Temporary, Full Time Join Amplius as a Community Outreach Support Worker Mental Health and make a real difference in our communities. You ll support customers with profound and enduring mental health conditions to sustain their tenancies, build independence, and improve their quality of life, delivering person-centred support through structured and community outreach services. Salary: £25,877.80 per annum Contract: Fixed Term Contract Up to 12 Months Your week: 37 hours per week Working to a 3-week rolling rota, between the hours of 8:30am and 8:30pm. This role does include 12-hour shifts, evenings and weekends. Location: Eastfield Road, Peterborough Snapshot of your role Provide ongoing, person-centred support to customers with profound and enduring mental health conditions to improve quality of life and promote independent living. Deliver a structured, flexible community outreach service tailored to individual needs. Support customers to sustain their tenancies, including understanding tenancy responsibilities such as property care and rent payments. Create and maintain a safe, supportive, and empowering environment for customers. Work closely with customers to build confidence, independence, and community integration. Liaise effectively with statutory and voluntary agencies, including Mental Health Teams and Adult Social Care, to deliver coordinated support. Contribute to ..... full job details .....
