You will manage a community caseload, completing home visits, follow-up assessments and rehabilitation interventions to support people to maintain or regain their independence. The caseload is approximately 55 active patients. Although this can sound high, the cases are not overly complex and existing members of the team are currently managing this level of caseload successfully. Initially, you will need to be happy to start and finish your working day at the team''s main base in Brent.
