Job Title: Community Link Worker Social Prescribing
Location: Derby (Derby City South Primary Care Network)
Salary: £25,750 pro rata
Contract Type: Permanent (subject to continuation of funding)
Hours: Part-Time 33.3 hours per week (or 9 days across a fortnight)
Working Pattern: Monday to Friday, with occasional evening and weekend working (time off in lieu provided)
