An exciting opportunity has arisen for a

Community Learning Disability Nurse

to work within the Intensive Community Assessment and Treatment Team (ICATT) based in Mansfield, Nottinghamshire. The Intellectual Disabilities Service, Specialist Services Directorate provides specialist NHS services to people for whom mainstream services cannot meet their needs. We are seeking a Nurse to join our well established Intensive Community Assessment and Treatment Team.

ICATT provides a rapid, short-term assessment and treatment service for people residing in the county of Nottinghamshire with a learning disability who may challenge services and may have additional mental health needs.

The ICATT Team seek to provide an alternative to hospital admission where it is appropriate to do so, enabling service users to receive treatment and care within their own homes, remain involved in their care as part of their own community and therefore minimising the disruption of their lives and the lives of those caring for them.

The successful candidate must be an

RNLD

with a Diploma or Degree level in Learning Disability Nursing and be registered with the NMC.

You will be highly motivated team player able to demonstrate a high level of professional competence. You will be expected to also work autonomously and be responsible for your own caseload. Excellent interpersonal and communication skills are essential. The post requires close work with patients, carers, external agencies and other health and social care professionals. As part of a team you will be expected to contribute to the risk management plans.

You must have the ability to analyse and assist in making decisions regarding client risk and support in complex cases.

You will be required to manage a busy caseload and carry out relevant physical and mental health assessments. You will provide assessment and treatment for service users living in the community and work closely with our local inpatient service.

You will participate fully as a multidisciplinary team member working in partnership with social care and voluntary/independent sector providers. You will provide holistic advice and health education to service users, their carers and community support agencies. The post holder must be able to demonstrate awareness of current issues and legislation affecting people with learning disabilities and or Autism.

Responsibilities

Manage your own caseload and work autonomously as part of the ICATT multidisciplinary team.

Provide assessment and treatment for service users living in the community; carry out relevant physical and mental health assessments; work closely with local inpatient services.

Participate fully as a multidisciplinary team member, in partnership with social care and voluntary/independent sector providers.

Contribute to risk management plans; analyse and assist in making decisions regarding client risk and support in complex cases.

Provide holistic advice and health education to service users, their carers and community support agencies.

Demonstrate awareness of current issues and legislation affecting people with learning disabilities and autism.

Qualifications

Registered NMC-licensed Learning Disability Nurse (RNLD) with a Diploma or Degree in Learning Disability Nursing; must be registered with the Nursing and Midwifery Council (NMC).

Ability to work as a highly motivated team player, demonstrate a high level of professional competence, and work autonomously with responsibility for own caseload.

Excellent interpersonal and communication skills; ability to work with patients, carers, external agencies and other health and social care professionals; contribute to risk management.

Ability to analyse and assist in decisions regarding client risk and support in complex cases.

Capacity to manage a busy caseload and carry out relevant physical and mental health assessments; provide community-based assessments and treatments; collaborate with local inpatient services.

Awareness of current issues and legislation affecting people with learning disabilities and/or autism.
