Community Learning Disability Nurse

Overview

The post holder will work in the Bexley Community Learning Disability nursing team based at Queen Mary’s Hospital, Sidcup, within the Learning Disability nursing service. The role focuses on the additional needs of people with complex physical health needs and requires management of a caseload of adults, working autonomously with a broad spectrum of physical health issues. The post holder will provide holistic, evidence-based nursing care, assessments and interventions, and will support students and junior staff to facilitate learning.They will work collaboratively as part of a multidisciplinary team and in partnership with other agencies/services, lead on providing robust health liaison to acute and primary care to ensure service users can access mainstream health services, and maintain an awareness of diversity and multi-cultural community issues.Responsibilities

Provide evidence-based nursing care to promote physical well-being, using LD-specific and mainstream health targets.Utilise a range of specialist assessments to identify health needs and plan appropriate interventions.Assess, implement, and critically evaluate intervention outcomes to maintain and improve physical health and well-being for service users.Deliver and delegate care delivery as appropriate to meet the health needs of people with learning disabilities.Collaborate with colleagues, carers, professionals and agencies around complex clinical interventions and outcomes.Assess and manage clinical risk for service users with complex physical health needs.Implement NHS health care policies and strategies in nursing practice and understand local and national policy agendas (e.g., Long Term Plan, Valuing People).Develop understanding of local complex physical health care pathways and clinical patient information and recording systems (RIO).Engage in health promotion with individuals and groups, providing education on various health needs and long-term conditions.Provide supervision to junior staff and students, with regular supervision from line management.Lead on health liaison activities to ensure access to mainstream health services.Qualifications and Experience

Essential: Qualified RNLD or RMNH.Desirable: BSc in Community Nursing or specialist qualifications related to the role.Essential: Sufficient post-registration experience working with people with learning disabilities; direct experience of facilitating group work; experience of supervising junior staff and students.Desirable: Experience of working in a multidisciplinary/multi-agency team.Skills/Ability and Knowledge: Demonstrate clinical skills appropriate to the role; desirable computer literacy and ability to use computer systems relevant to the role.Additional Information
