Community IV NurseThe Role As a Community IV Nurse, you will provide high-quality, patient-centred intravenous therapy and specialist nursing care to patients in their own homes and community settings. You will support patients throughout their treatment journey, from therapy initiation and assessment through to ongoing management and completion of treatment. This will include delivering IV therapies safely, providing education to patients, families and carers, monitoring treatment response, and supporting patients to manage their care as independently as possible. Working closely with pharmacists, consultants, specialist nurses and other healthcare professionals, you will play an important role in delivering safe and effective care outside of the hospital environment. You will manage your own community caseload, plan and undertake patient visits, maintain accurate clinical records, and contribute to service quality through audit, governance and continuous improvement. Key Responsibilities Deliver safe and effective IV therapies in patients'' homes and community settings in accordance with Service Operating Procedures, clinical guidelines and prescribed treatment plans. Undertake patient assessments and provide ongoing monitoring throughout the course of treatment. Administer prescribed intravenous medications and therapies safely and effectively.
