Community Dietitian - Band 6 Location: Hillingdon Community Services Central and North West London NHS Foundation Trust (CNWL)Contract Type: Bank / AgencySalary: Competitive Hourly RateStart Date: As soon as possible About the Role We are recruiting a Band 6 Community Dietitian to join the Community Adult Rehabilitation Service within CNWL. This is an excellent opportunity to work as part of a supportive multidisciplinary team, delivering specialist nutritional support to adults within their own homes, care homes, and intermediate care settings. Key Responsibilities Provide specialist dietetic assessment and treatment for adults requiring nutritional support, including Home Enteral Tube Feeding (HETF) Deliver evidence-based nutritional interventions for patients who are malnourished, at risk of malnutrition, or enterally fed Assess and calculate nutritional requirements, including food fortification, oral nutritional supplements, and enteral feeding regimens Provide dietary counselling and support behaviour change to patients, families, and carers Make recommendations to GPs regarding appropriate prescribing of oral nutritional supplements and enteral feeds Participate in MDT meetings, discharge planning, and collaborative community care pathways Maintain accurate clinical documentation and caseload management Support training and education for healthcare staff, carers, and students Contribute to audit, service development, and quality improvement ..... full job details .....
