Community Care Coordinator

About the Role We are looking for a Care Coordinator to join our team and help ensure our care services are safe, well-coordinated and consistently delivered to a high standard. You will work closely with the people we support, their families, care workers, healthcare professionals, commissioners and the Registered Manager. You will play an important role in coordinating care packages, supporting staff and maintaining excellent standards of care and compliance. This role would suit someone who is organised, compassionate, confident and passionate about making a positive difference to people''s lives. Key Duties Coordinate care packages and allocate care workers according to individual needs. Maintain accurate care schedules, care plans and service documentation. Work with PWS, families and professionals to develop and review person-centred care plans. Respond to staff absences and emergencies, ensuring continuity of care.
