OverviewTo assist with the direct care of patients as delegated to and supervised by a registered nurse/health care professional, ensuring safe and compassionate care within defined competencies.

Responsibilities

Work under supervision of a registered practitioner or a Level 4 Associate Practitioner, with supervision that may be remote or indirect.

Provide direct patient care and support to the nursing team, taking responsibility for well-defined routine clinical and non-clinical duties within the limit of competence.

Collaborate with the multi-disciplinary team to deliver high quality compassionate care.

Uphold safety and quality standards, putting patients first and protecting them from harm.

Clinical duties and activities

Assist with delivery of individualised holistic patient care guided by standard operating procedures, protocols, systems of work and policies.

Obtain informed consent prior to delivering care.

Interact with relatives/carers to provide information and involve them in care.

Ensure care is delivered with compassion and respect; protect privacy and confidentiality.

Participate in risk assessments with the multi-disciplinary team (e.g., pressure ulcers, falls, nutrition) to prevent patient harm.

Accurately measure and record physiological observations (pulse, respirations, blood pressure, temperature) and escalate as needed.

Measure and record blood glucose levels and report to the supervising clinician.

Document observations and care provided in the patient record; inform the delegated nurse of actions taken.

Undertake urinalysis, record results, and report to the patient’s nurse/doctor.

Safely obtain specimens (blood, urine, stool, sputum, etc.) in line with infection prevention policies; ensure accurate documentation.

Transport samples to laboratories promptly and professionally.

Respond to patient questions and assist in evaluating care.

Use appropriate moving and handling techniques and equipment for safe patient handling.

Report relevant information to the responsible registered nurse.

Patient experience and environment

Make patient care your first concern and aim for a positive patient experience.

Answer telephone calls professionally, take messages accurately and escalate concerns as needed.

Maintain a clean, safe environment; comply with health, safety, infection prevention, and organizational policies.

Ensure clinical areas are stocked at appropriate levels and equipment is properly stored.

Adhere to Health and Safety regulations and report accidents or hazards promptly using the incident reporting system.

Professional behaviour and development

Maintain courteous, respectful professional behaviour; support compassionate care and the care of deceased patients and families where appropriate.

Act as a representative of the Trust and embody its values; pursue ongoing professional development and self-directed learning.

Work towards registration as a healthcare support worker with the Nursing and Midwifery Council as established.
