Location: Bodmin, Cornwall
Salary: Up to -56,000 per annum

Job Summary
Healthii People are recruiting on behalf of a well-established Primary Care Network (PCN) for a Clinical Pharmacist to join their pharmacy team.
This role involves working across neighbourhood practices, providing high-quality clinical support within general practice, and being an integral part of a multidisciplinary team (MDT). Flexible working arrangements are available, with opportunities for both part-time and full-time contracts, and the option to work as a hybrid role with some remote working.
The successful candidate will:
- Undertake clinical medication reviews, particularly for patients with complex polypharmacy
- Support care homes and patient home visits
- Manage elements of the Quality and Outcomes Framework (QOF) and enhanced services
- Advise practice staff on prescription and medication queries
- Represent the PCN in medicines optimisation and quality improvement initiatives
You will receive clinical supervision and support to develop into a non-medical prescriber if required.
Main Duties of the Role
- Conduct structured medication reviews
- Provide support to care homes and community settings
- Collaborate as part of a large MDT
- Contribute to patient safety and quality improvement projects
- Integrate services with community and hospital pharmacy teams

Person Specification
Essential:
- MPharm / Undergraduate degree in Pharmacy
- Registered with the General Pharmaceutical Council (GPhC)
- Strong interest in long-term condition management, patient safety, and medication optimisation
- Commitment to working within an MDT approach
Desirable:
- Experience of working in primary care
- Experience of working in a GP practice
This role offers up to -56,000 per annum, part-time or full-time hours, the flexibility of a hybrid role, career progression, supportive supervision, and the opportunity to qualify as a non-medical prescriber.
