Occupational TherapistLocation: Thackley Green Specialist Care Centre, North NorthamptonshireHours: 37 hours per weekAbout the RoleWe are seeking a motivated and compassionate Occupational Therapist to join our team at Thackley Green Specialist Care Centre. This is an exciting opportunity to play a key role in supporting patient recovery and enabling safe, timely discharges from hospital settings.You will lead therapeutic assessments and rehabilitation for patients transitioning from hospital into our service, working collaboratively within a multidisciplinary team (MDT) to deliver person-centred care and achieve positive outcomes.Key ResponsibilitiesLead comprehensive therapy assessments and rehabilitation programmes for patients admitted from hospitalConduct home access visits to assess the need for minor adaptations and equipmentEffectively manage your own caseload, prioritising and planning interventions to meet patient needsDelegate tasks appropriately to Therapy Assistants, ensuring safe and efficient service deliverySupport the wider Discharge to Assess pathway, taking on referrals where requiredWork closely with multidisciplinary professionals to facilitate smooth and timely dischargesPromote independence and support individuals to return safely to their home environmentFull UK driving licence with access to a vehicle and appropriate business insurance (essentialIf this is the role for you then we would love to hear from you. ..... full job details .....
