Clinical Lead Nurse

Cityworx are recruiting on behalf of our client, an established and trusted home care provider, who are looking for an experienced Registered Nurse to join their team as a Clinical Lead and help build and lead a new nurse-led clinical care service across Berkshire.

This is an exciting opportunity for an experienced Registered Nurse to play a key role in developing a new clinical service from the ground up. Working across Reading, Wokingham, Windsor, Maidenhead and Bracknell, the Clinical Lead will combine strong clinical leadership with relationship-building and service development to ensure the delivery of safe, effective and person-centred care.

The successful candidate will have the opportunity to shape the service, develop relationships with hospitals, ICBs, commissioners, case managers, discharge teams and other healthcare professionals, and help establish the systems and processes required to support a high-quality clinical service.

Key Responsibilities:

• Help establish and grow a nurse-led clinical care service for people with complex care needs.

• Build and maintain strong professional relationships with ICBs, hospitals, commissioners, case managers, discharge teams and other healthcare professionals.

• Develop relationships with referral partners to support a sustainable pipeline of new care opportunities.

• Assess customers and identify their individual clinical and care needs.

• Develop and review person-centred care plans.

• Provide clinical leadership to ensure safe, effective and high-quality care delivery.

• Support the recruitment, development and ongoing support of the growing clinical team.

• Develop appropriate systems, processes and clinical governance arrangements for the new service.

• Support the organisation with its registration for TDDI with CQC.

• Work collaboratively with the wider leadership team to develop and grow the clinical service.

• Contribute clinical expertise while also demonstrating an understanding of the commercial aspects of developing a new service.

• Work autonomously while ensuring that appropriate clinical standards, governance and processes are maintained.

• Identify opportunities to improve and develop the service and contribute ideas to its future direction.

For this role, we are looking for an experienced Registered Nurse with experience in complex care, community nursing, case management or a similar clinical setting. You will need to be confident working autonomously, building professional relationships and providing strong clinical leadership-to help develop a new service.
