Overseeing client assessments, reassessments, care planning, case management and clinical reviews. Ensuring appropriate skill mix, continuity of care and client safety across service delivery. Review client schedules for suitability, making amendments where safety or quality risks arise. Conducting routine, urgent and complex client visits as required.
