Do not miss out on this fantastic opportunity!Responsibilities of this role:Manage a caseload of children and young people with disabilities, ensuring their needs are assessed and support plans are implemented effectively.Undertake statutory assessments, Child in Need, Child Protection, and Looked After Children responsibilities in line with legislation and statutory guidance.Work directly with children, young people, and their families to identify strengths, needs, and desired outcomes, ensuring their voices inform planning and decision-making.Develop and review care plans, support packages, and interventions that promote safety, wellbeing, independence, and positive outcomes.Collaborate with education, health, and other partner agencies to provide coordinated, multi-agency support for children and families.Identify, assess, and manage safeguarding risks, taking appropriate action to protect vulnerable children and young people.Attend and contribute to multi-agency meetings, reviews, and conferences, providing professional advice and recommendations.Maintain accurate and timely case records, assessments, and reports, ensuring compliance with statutory requirements and professional standards.Requirements of this role:Must be a Qualified Social ..... full job details .....
