Working within the Children and Education Brokerage and Placements Team, you will provide specialist oversight and coordination of Children''s Continuing Care referrals, supporting children, young people and families through every stage of the pathway. This includes identification, triage, referral, assessment, panel presentation, funding agreement, monitoring and review activities.
