      <description>Location: Telford | Salary: Negotiable | Type: Contract | Our client Telford and Wrekin Council is looking for a Children''s Advanced Practitioner to join their Social care team Team.  Working Arrangements  Hybrid working, there will be an expectation that there will be a mixture of office based and home working as agreed with the team manager  Duration: 3 months with potential extension  Key Responsibilities  Manage a diverse caseload, undertaking timely assessments, care planning and interventions for children and families.  Respond to referrals and safeguarding concerns, assessing risk and making evidence-based decisions.  Undertake Child and Family Assessments, contributing to child protection processes and legal proceedings where required.  Build effective relationships with children, families and multi-agency partners to inform assessment and planning.  Apply legislation, policy and best practice to deliver high-quality, child-centred services.  Use professional judgement to identify appropriate interventions, review progress and adapt plans where necessary.
