About the RoleWe are looking for a compassionate and motivated Recovery Coordinator to join our specialist substance misuse service. You will support individuals affected by drug and alcohol dependency, helping them achieve sustainable recovery through person-centred care planning, risk management, and partnership working.This is an excellent opportunity for someone passionate about improving lives and making a real difference within the community.Key ResponsibilitiesManage a caseload of service users from assessment through to recovery and discharge.Complete comprehensive assessments and develop personalised recovery plans.Deliver ongoing key working sessions and regular reviews.Coordinate interventions tailored to individual needs.Signpost and refer service users to specialist services including:HousingMental HealthEmployment and EducationBenefits AdvicePhysical Health ServicesPromote harm reduction and safer substance use practices.Carry out risk assessments and safeguarding procedures.Maintain accurate electronic case records.Work closely with multi-disciplinary teams and external agencies.Encourage service users to build resilience, independence and long-term recovery.Support individuals to access community recovery groups and peer support networks.Ensure all work complies with safeguarding, confidentiality and GDPR requirements.Essential RequirementsExperience working within substance misuse, mental health, homelessness, criminal justice or a related support ..... full job details .....
