This is an excellent opportunity for an experienced support professional to provide practical and person-centred support to carers of individuals with learning disabilities and neurodiverse needs. Working within a community-focused service, you will manage a caseload of carers, provide tailored advice and guidance, facilitate peer support activities, and connect individuals with the services and resources they need to thrive. Key Responsibilities  Manage referrals and undertake assessments to identify the support needs of carers. Provide one-to-one advice, guidance, signposting and ongoing casework support.
