As a Care Navigator with PRIMARY CARE experience, you will be providing a vital link between patients, doctors, and other healthcare professionals. Job Summary: We are looking for a skilled and dedicated Care Navigator to join our team on a contract basis. As a Care Navigator, you will be responsible for providing administrative support to GP Practices and Primary Care Hubs, ensuring that patients receive the best possible care. If you have previous experience as a Receptionist or Care Navigator within a GP or health centre, we would love to hear from you.
