Care Navigator Salary: £28,353 - £31,756 per annum + 6% Hours: 36 hours per week Based at: Haringey Multi Agency Care and Coordination (MACC) Team Contract: One year (possible extension) Closing date: 12 October 2026, 12 noon Interviews: w/c 12 October 2026 The Bridge Renewal Trust, in partnership with the NHS, are delivering care navigation services for frail patients in Haringey. As a Care Navigator, you will work as part of a multidisciplinary team to support frail patients in Haringey by providing individual holistic assessments and directing patients to appropriate services, including activities that may help to promote their health, wellbeing and independence.
