Care Manager

Job-Scope:

We-are-looking-to-recruit-a-Care-Manager-to-lead-and-develop-complex-care-services-within-a-community-setting.-As-a-Care-Manager,-you-will-oversee-a-caseload-of-clients-with-complex-health-needs,-ensuring-that-every-care-package-is-safe,-person-centred,-and-responsive-to-changing-requirements.

The-Care-Manager-will-take-ownership-of-transitions,-start-ups,-and-the-ongoing-management-of-care-packages,-working-closely-with-families-and-multi-disciplinary-teams-to-deliver-the-highest-standards-of-support.-You-will-regularly-review-care-plans-and-risk-assessments,-maintaining-full-compliance-with-safeguarding-and-regulatory-standards.

In-this-role,-the-Care-Manager-will-also-provide-leadership,-mentoring,-and-development-opportunities-for-care-teams,-ensuring-staff-are-trained-and-confident-to-deliver-exceptional-care-in-the-community.-This-is-an-opportunity-to-make-a-real-difference-by-shaping-high-quality,-compassionate-services-that-empower-clients-and-their-families.

Do-you-have?

• Experience-in-managing-complex-care-packages,-including-conditions-such-as-spinal-injuries,-acquired-brain-injuries,-motor-neuron-disease,-or-multiple-sclerosis
• Strong-knowledge-of-CQC-regulations,-compliance,-and-safeguarding-practices
• Proven-leadership-skills-with-the-ability-to-motivate-and-mentor-care-teams
• Excellent-organisational-and-time-management-abilities
• A-people-first-approach-with-passion-for-delivering-exceptional-care
• A-full-driving-licence-and-access-to-your-own-car
