Care Coordinator
Care Delivery Practitioner - The key purpose of the Care Delivery Practitioner ( care co-ordinator) role is to support a growing Community Care team in our Leeds service (CQC) managing the candidate pool including the rota/shift process. Ensuring that the personal care, independence, and choices of the Service User are maintained, according to the needs and capabilities of the individual, as recorded in the care and support plans. This role also encompasses the support of service delivery requirements to maintain the highest standard of care and quality of service.
