An exciting and new opportunity has arisen for a Care Coordinator to work within the Early Intervention in Psychosis Team in Bolton.We are looking for an experienced and enthusiastic Registered Mental Health Nurse who has a passion for providing support during the first episode of psychosis to promote mental and physical health, wellbeing, and recovery.The purpose of the role is to reduce the stigma associated with psychosis, provide early intervention to decrease the duration of undiagnosed and untreated cases, and offer support and guidance throughout the recovery journey of individuals.You will join a client-centred, dynamic, and friendly team that prides itself on delivering high-quality, evidence-based interventions for those experiencing first-episode psychosis.Your responsibilities will include providing effective care coordination to a caseload of complex service users, ensuring the delivery of evidence-based interventions to promote successful outcomes under the Care Programme Approach (CPA).You will support the wider team in duty and assessment tasks and demonstrate effective communication with clients, carers, and other internal and external agencies.Greater Manchester Mental Health (GMMH) Foundation Trust employs over 7,000 staff members, delivering services from more than 122 locations.We provide inpatient and community-based mental health care for people living in Bolton, Manchester, Salford, Trafford, and Wigan, along with a wide range of specialist mental health and substance misuse services across Greater Manchester, the North West of England, and beyond.Greater Manchester is one of the world''s most innovative and vibrant places to live and work.
