This is an exciting opportunity for a dedicated Care Coordinator who is passionate about delivering high-quality, person-centred care and supporting the smooth day-to-day coordination of care services. As a Care Coordinator, you will play a key role in ensuring care packages are staffed effectively, rotas are managed efficiently, and both clients and care staff receive excellent support.
