Job Title: Care Coordinator

Salary: -27,000 per annum + on-call bonus
We are currently seeking a highly organized and compassionate Care Coordinator to join our busy domiciliary care agency based in Wymondham. The ideal candidate will have excellent communication and problem-solving skills, as well as a strong understanding of the care industry. This is a full-time position with the potential for growth within the company.
Key Responsibilities:
Coordinating and managing the care schedules for clients, ensuring all visits are covered and caregivers are allocated appropriatelyLiaising with clients, caregivers, and other healthcare professionals to ensure the highest quality of care is providedConducting regular assessments and reviews of clients' care needs and updating care plans accordinglyParticipating in the recruitment and training of caregivers to maintain a strong and reliable teamManaging and maintaining accurate records and documentation in line with company policies and proceduresOn-call responsibilities, including responding to emergencies and providing support to caregivers and clients outside of office hoursRequirements:
Previous experience in a care coordination or similar role within the healthcare industryExcellent communication and interpersonal skills, with the ability to build strong relationships with clients, caregivers, and other healthcare professionalsStrong organizational and time-management skills, with the ability to prioritize tasks and manage a busy workloadKnowledge of relevant legislation and regulations within the care industryProficient in using computer systems and software, with the ability to learn new systems quicklyA compassionate and empathetic nature, with a genuine passion for providing high-quality care to vulnerable individualsIf you are looking for a rewarding and challenging role within the care industry, and have the necessary skills and experience, we would love to hear from you.
