Role Overview:
We are seeking a highly motivated and experienced Care Coordinator to join our team. The ideal candidate will have excellent communication and organizational skills, with a passion for delivering exceptional care services. This is an exciting opportunity for a Team Leader or administrator looking to progress into a coordinator role.
Key Responsibilities:
Coordinate and manage the delivery of care services to clients in their own homesConduct initial assessments and develop care plans in collaboration with clients and their familiesEnsure that all care services are delivered in accordance with company policies and procedures, and regulatory standardsMaintain accurate and up-to-date client records and care plansMonitor and review the quality of care provided, and make necessary improvementsLiaise with healthcare professionals, families, and other stakeholders to ensure the best possible care for clientsManage a team of care staff, including recruitment, training, and performance managementParticipate in on-call duties as requiredRequirements:
Previous experience in a care coordination or similar roleNVQ Level 3 in Health and Social Care or equivalent would be useful but not essentialWould consider an administrator looking to move into a coordinator roleExcellent communication and interpersonal skillsStrong organizational and time-management skillsKnowledge of relevant legislation and regulations in the care sectorAbility to work independently and as part of a teamProficient in using computer systems and softwareWillingness to work flexible hours, including occasional weekends and eveningsBenefits:
