Join to apply for the Care Coordinator - CPN role at Greater Manchester West Mental Health NHS Foundation Trust. This is an exciting opportunity for a Qualified Mental Health Nurse, Social Worker or Occupational Therapist to join our specialist service for people experiencing a first episode of psychosis as a Care Co-ordinator.Responsibilities
Use sound clinical and risk assessment skills to plan and deliver interventions to high standards.Provide evidence-based interventions for young people experiencing a first episode of psychosis and promote recovery during the early stage of illness.Develop meaningful engagement with service users and their families and work in partnership with other services (primary care, education, social services, youth, and more).Ensure care is transferred thoughtfully and effectively at the end of involvement.Contribute to staff training to ensure knowledge and skills align with NICE concordant packages of care and Best Practice recommendations.Qualifications
Qualified Mental Health Nurse, Social Worker or Occupational Therapist.Genuine interest in providing evidence-based interventions for young people with a first episode of psychosis.Demonstrated ability to complete high-standard clinical assessments and interventions.Requirements
Experience in early intervention services or related mental health settings is desirable.Ability to work within an integrated service model for individuals aged 14 to 65 years.Benefits
