We provide high-quality home care and community services to vulnerable Adult and Children.Summary of role: Care CoordinatorThe Care Coordinator is to generate and promote a caring and supportive environment for CareFocal''s clients by implementing very high standards of professional care and support. To manage the staff team, supporting their development, skills and abilities.Principal responsibilities: Care CoordinatorThe Care Coordinator will provide good service to clients by ensuring that prior to each service commencing, a risk assessment with the client, and/or their chosen representatives, has been completed including what the client needs and would like to achieve from their care and support.The Care Coordinator will implement quality management and improvement systems. Effectively manage complaints and incidents. Carry out investigations relating to the quality of the service and use findings to make improvements.The Care Coordinator will support Staff Recruitment, Induction and Training. Provide information, guidance and on-going supervision to enable staff to effectively and safely carry out their roles.
