Care Coordinator

An excellent opportunity has become available for an experienced Care Coordinator to join a well-established, privately owned domiciliary care provider in Eastbourne.Working alongside the Registered Manager, you will play a key role in ensuring the delivery of safe, high-quality, person-centred care. This is a varied role that includes managing staff and client rotas, coordinating care packages, completing client assessments and person-centred care plans, and maintaining compliance with CQC regulations. You will also provide support within the community when required and participate in the on-call rota.This opportunity would suit someone with previous experience as a Care Coordinator or in a similar role within domiciliary care who enjoys building relationships, supporting care teams and delivering excellent customer service.RequirementsPrevious experience as a Care Coordinator or similar within domiciliary care. Good understanding of CQC regulations and person-centred care planning. Experience completing client assessments and care plans. Strong communication, organisational and IT skills. Willingness to undertake care visits when required. ResponsibilitiesSupporting the Registered Manager with the day-to-day coordination of the service.
