CARE COORDINATOR MILTON KEYNES - £25500 - £28500 NO SPONSORSHIP OFFERED, MUST HAVE RIGHT TO WORK IN UK AND MUST HAVE UK DRIVERS LICENCE AND OWN CAR The Care Coordinator is responsible for coordinating and overseeing the day-to-day delivery of domiciliary care services to ensure that clients receive safe, effective, person-centred and consistent care. The role involves coordinating care packages, preparing and maintaining staff rotas, communicating with clients, families and care workers, monitoring care delivery, responding to changes in clients'' needs, and supporting the Registered Manager in maintaining high standards of care and regulatory compliance. The Care Coordinator will work closely with care staff and management to ensure that the right care worker is available at the right time and that care is delivered in accordance with each client''s care plan and agreed outcomes. Key Responsibilities Coordinate the day-to-day delivery of domiciliary care services. Arrange and maintain care worker rotas to ensure all scheduled visits are appropriately covered. Allocate care workers based on client needs, skills, availability, continuity of care and geographical considerations. Respond promptly to changes in care requirements, cancellations, sickness and emergencies. Arrange suitable cover for unplanned staff absence.
