The role involves coordinating care packages, preparing and maintaining staff rotas, communicating with clients, families and care workers, monitoring care delivery, responding to changes in clients' needs, and supporting the Registered Manager in maintaining high standards of care and regulatory compliance. The Care Coordinator will work closely with care staff and management to ensure that the right care worker is available at the right time and that care is delivered in accordance with each client's care plan and agreed outcomes.
