This is a full-time role working 37 hours per week, Monday to Friday. Key Responsibilities Undertake assessments of vulnerable adults and contribute to support planning in line with Adult Social Care legislation and best practice. Carry out allocated care management duties, ensuring assessments and reviews are completed to a high standard. Develop person-centred support plans that promote independence, choice, dignity and wellbeing. Work closely with service users, families and carers, ensuring they are fully involved in the assessment and care planning process. Promote self-directed support, enabling individuals to access appropriate community resources and services. Support individuals in maintaining their health, wellbeing, choice and control, with full regard to mental capacity. Maintain accurate records and contribute to the continuous development of the team and service.
