Our client, a well-established provider of live-in care services, is looking for a Care Coordination Consultant to join their friendly and supportive team in Tonbridge. You''ll play a vital role in matching self-employed live-in carers with clients, ensuring continuity of care and creating positive experiences for everyone involved.
This is much more than scheduling and administration. It''s about building relationships, solving problems, supporting carers, and making sure vulnerable people receive the right care at the right time.
What You''ll Be Doing You''ll be responsible for coordinating live-in care services from start to finish, including:
- Building strong relationships with clients, families and carers.
- Matching carers to clients based on skills, experience and personality.
- Managing schedules and ensuring continuity of care.
- Supporting carers throughout their placements.
- Resolving challenges quickly and finding practical solutions.
- Maintaining accurate records using CRM systems.
- Working closely with internal teams to ensure a seamless service.
- Supporting service improvements and contributing ideas to enhance efficiency.
- Participating in the out-of-hours on-call rota.
Essential Skills & Experience:
- Excellent communication and relationship-building skills.
- Strong organisational and time-management abilities.
- The ability to manage multiple priorities in a busy environment.
- High levels of accuracy and attention to detail.
- Strong problem-solving and decision-making skills.
- A customer-focused approach.
- Good IT skills, including experience using CRM systems and Microsoft Office.
- Experience within health and social care.
- Knowledge of live-in care services.
- Understanding of safeguarding, GDPR and CQC requirements.
- Experience coordinating schedules, rotas or workforce planning.
- Positive, approachable and supportive.
