Duties and Responsibilities as a care assistant : Provide person-centred support to vulnerable adults, promoting independence, dignity, and wellbeing. Build positive relationships with service users to understand their individual needs, preferences, and goals. Develop, review, and maintain support plans and care plans, ensuring they reflect each individual s changing needs and circumstances. Complete annual support plan reviews and contribute to multi-agency meetings where required. Monitor and support mental health and emotional wellbeing, recognising early signs of deterioration and responding appropriately. Safeguard service users by identifying and reporting any concerns, abuse, or neglect in line with safeguarding policies and procedures. Carry out regular health and safety checks within the service environment to ensure a safe and supportive setting. Support with daily living activities, including budgeting, medication prompts, personal care, and access to community services.
