Duties and Responsibilities Manage an allocated caseload of adults experiencing mental health needs. Undertake comprehensive assessments, reviews, and care and support planning in line with the Care Act 2014. Assess risk and identify appropriate interventions and support to promote independence and wellbeing. Undertake Mental Capacity Assessments and Best Interests decision-making where required. Respond appropriately to safeguarding concerns and work in accordance with statutory requirements. Work collaboratively with service users, carers, health professionals, housing, and other partner agencies. Develop person-centred, outcome-focused support plans based on individual needs and strengths. Maintain accurate case records, assessments, reviews, and reports. Work effectively within a multi-disciplinary environment to achieve positive outcomes for service users.
