Brokerage OfficerSalary: £26.96 umbrellaLocation: Civic Centre, High Street, Uxbridge, Middlesex, UB8 1UW About the RoleWe are looking for an experienced and proactive Brokerage Officer to join our team and play a key role in sourcing and coordinating high-quality, cost-effective care and support services. The successful candidate will be responsible for arranging a range of care provisions, including home care packages, residential and nursing placements, respite care and emergency placements. You will work closely with frontline practitioners, senior managers and external care providers to ensure that services are sourced and arranged in line with identified needs and Care Plans. This is an excellent opportunity for someone with experience in Adult Social Care who understands the complexities of care commissioning and brokerage and can work confidently in a fast-paced environment. Experience of using the LAS (Liquidlogic Adult Social Care) system is desirable. Main Duties and ResponsibilitiesPurchase Home Care hours in accordance with the stated needs identified and outlined within the Care Plan.Purchase Residential and Nursing Care placements, including respite care, in accordance with assessed needs and Care Plans.Ensure appropriate levels of care provision are available to support the Emergency Duty Team (EDT), including emergency beds and home care hours.Ensure that block contracts are utilised effectively and to their optimum capacity.Develop and maintain effective ..... full job details .....
