Brokerage Officer

We''re looking for an experienced Adults Brokerage Officer to join Hackney Council''s Commissioning Team.This is a great opportunity for someone with experience within Adult Social Care brokerage, commissioning or placement finding who is confident working with care providers and internal teams to secure high-quality, value-for-money care and support.The RoleAs a Brokerage Officer, you''ll work closely with Service Managers, Senior Practitioners and Care Managers to source, arrange and amend packages of care. This will include placements within residential and nursing homes and supported living services, supporting adults including older people, individuals with learning disabilities and those accessing mental health services. Key responsibilities will include:Sourcing appropriate Adult Social Care placements and packages of careLiaising and negotiating with care providers to achieve value for moneySupporting supplier set-up, contracting and amendmentsWorking closely with Care Management, Hospital Discharge, Finance and Information and Assessment teamsMaintaining accurate records and updating Mosaic and CM2000Supporting the identification of service gaps and potential new providersMaintaining up-to-date knowledge and intelligence of the local care marketContributing to commissioning and market management activitySupporting person-centred services that promote choice, independence and positive outcomes.About YouWe''re particularly interested in candidates who have:Experience ..... full job details .....
