This is an excellent opportunity for someone who is passionate about delivering person-centred care, working collaboratively with individuals and families, and sourcing high-quality, cost-effective care and support solutions.About the RoleThe Brokerage Service is responsible for planning, sourcing, negotiating and arranging care and support packages that meet the individual needs of adults and young people transitioning into adult services. This includes arranging specialist placements, coordinating packages of care, and working with approved providers, framework agreements and block contracts. As a Brokerage Officer, you will work alongside individuals, recognising them as the experts in their own lives. You will help identify the most appropriate support by promoting independence, making the best use of community resources, and ensuring services are both person-centred and financially sustainable.Key Responsibilities Arrange personalised, cost-effective care and support packages.Work in partnership with individuals, families and professionals to co-produce support plans.Source services through approved providers, framework agreements and local community ..... full job details .....
