Hours: Full-time, Monday to Friday
Client: NHS Cheshire and Merseyside Integrated Care Board (ICB)
Essential: Experience of brokering, sourcing or commissioning care packages or placements, strong organisational and communication skills, knowledge of the health and social care sector, and the Right to Work in the UK.
Job Summary
We''re currently supporting Cheshire and Merseyside ICB in recruiting an All Age Continuing Care (AACC) Broker Administrator to join the Sefton and Liverpool AACC Team.
This is an excellent opportunity for an experienced administrator, broker, care coordinator or commissioning professional to play a vital role in sourcing and coordinating care packages for individuals with complex health needs. You''ll work closely with healthcare professionals, care providers, local authorities and families to ensure timely and cost-effective care arrangements are in place.
The role offers the chance to make a real difference to patient outcomes by supporting hospital discharges, Continuing Healthcare services and ongoing care provision across the community.
Who We''re Looking For
You''re a highly organised professional with experience coordinating services within health and social care. You enjoy building relationships, managing competing priorities and finding effective solutions to complex situations.
You''ll be confident communicating with a range of stakeholders and able to work independently while maintaining accuracy and attention to detail in a fast-paced environment.
Key Responsibilities
- Source and broker packages of care for individuals eligible for Continuing Healthcare and related services.
- Liaise with care providers to identify suitable placements and care arrangements.
- Support hospital discharge pathways by securing timely care provisions.
- Negotiate rates and ensure value for money when commissioning services.
- Maintain accurate records and documentation relating to care packages.
- Work collaboratively with NHS teams, local authorities, care providers and families.
- Monitor provider availability and capacity.
- Support reporting, audit and financial processes within the service.
- Escalate concerns, risks or service issues where appropriate.
Essential Skills & Experience
- Experience in brokering, commissioning or coordinating care packages.
- Previous experience working within health or social care services.
- Strong organisational and administrative skills.
- Excellent communication and stakeholder management abilities.
- Ability to manage multiple tasks and meet deadlines.
- Experience maintaining accurate records and using IT systems.
- Strong problem-solving and negotiation skills.
Right to Work in the UK.
Desirable Skills & Experience
- Knowledge of NHS Continuing Healthcare (CHC).
- Experience working within NHS, local authority or integrated care settings.
- Understanding of care provider markets and commissioning processes.
- Experience supporting hospital discharge pathways.
- Knowledge of care funding and package approval processes.
