The successful candidate will support service users, carers, and families through person-centred, strengths-based interventions, helping individuals maximise their independence, choice, and control.This position is covering two staff members currently undertaking Social Work Apprentice Training.Key ResponsibilitiesConduct assessments, devise care and support plans, and review and monitor plans in line with Care Act responsibilities and other statutory frameworks.Participate in the team''s duty arrangements.Apply a strengths-based approach using the Conversations Model to connect people with their communities and social networks and support collaborative problem-solving.Work closely with individuals, advocates, and carers to develop person-centred support plans that are legal, affordable, and promote independence.Assess and support family carers, identifying their needs ..... full job details .....
