Bed Brokerage Project Lead

Bed Brokerage Project Lead - Medway Medway Council are looking to recruit a consultant to design, implement and introduce a Bed Brokerage team that sources care for patients funded by both health and social care. The council currently has a bed brokerage team dedicated to sourcing care for people being discharged from hospital, intermediate care beds and D2A beds that are funded by Adult Social Care. The intention is that the team will be expanded and going forward will source care for health funded patients. Bed Brokerage Project Lead responsibilities include:Lead on the development of a joint health and social care brokerage function.Develop and implement a joint funding model, ensuring legal and financial compliance.Project manage the full integration process, from planning to operational delivery.If applicable, identify the transfer process for affected staff, ensuring engagement, clear communication, and compliance with employment law.Engage stakeholders from social care, health, finance, HR, and commissioning teams to co-produce solutions and remove barriers.Design governance, performance, and reporting frameworks for the new team.Evaluate and manage risks throughout the transition and implementation phases. Minimum requirements / Experience preferred:Educated to degree level in project management or equivalent level qualificationEvidence of continuous professional developmentCompletion of qualification in change managementStrong experience in leading service ..... full job details .....
