OverviewBand 7 Clinical Lead - South Neighbourhood Mental Health Team . An exciting opportunity to join the Neighbourhood Mental Health Team working across South Birmingham within Primary Care Networks (PCNs).

We are looking for enthusiastic and dynamic individuals to work within a multi-disciplinary team, collaborating with other professionals and striving for the best outcomes for individuals within the local community.

Responsibilities

Provide mental health triage, assessment and outcomes-based intervention for service users within the Neighbourhood Mental Health Team.

Identify need and determine the best course of advice, support or intervention.

Promote psychosocial wellbeing by identifying and supporting pathways for service users and carers.

Ensure robust communication across FTB/BSMHFT and primary care in accordance with each service user''s needs; provide support and advice to all colleagues within the PCN as required, including participation within the local neighbourhood MDTs and those working in the voluntary sector.

Qualifications / Experience

Experience working within a multi-disciplinary mental health team.

Ability to work alongside GP colleagues within PCNs.

Strong communication and collaboration skills to support service users, carers and colleagues.
