Band 7 Children''s CHC Nurse AssessorWe are currently recruiting for an experienced Band 7 Children''s CHC Nurse Assessor to support an NHS Integrated Care Board in London.This is an excellent opportunity for an experienced Continuing Care professional to join a busy team responsible for assessing, reviewing and coordinating care packages for children and young people with complex health needs.Key Responsibilities:* Undertake Children''s Continuing Care assessments and reviews in line with the National Framework and local policies.* Assess the healthcare needs of children and young people with complex needs.* Produce high quality assessment reports and recommendations.* Participate in multidisciplinary team meetings and case discussions.* Work collaboratively with families, healthcare professionals, education providers and local authority colleagues.* Support care planning and ongoing review processes.* Ensure accurate and timely documentation is maintained.Essential Requirements:* Registered Nurse with current NMC registration.* Previous experience undertaking Children''s Continuing Care (CHC) assessments.* Strong understanding of Continuing Care processes and frameworks.* Experience working within NHS services.* Excellent communication, report writing and stakeholder management skills.* Ability to work autonomously and manage a varied caseload.Contract Details:* Band 7 Children''s CHC Nurse Assessor* London* £300 per day* Contract until September 2026* Full-time ..... full job details .....
