Band 7 Children''s CHC Nurse AssessorWe are currently recruiting for an experienced Band 7 Children''s CHC Nurse Assessor to support an NHS Integrated Care Board in London.This is an excellent opportunity for an experienced Continuing Care professional to join a busy team responsible for assessing, reviewing and coordinating care packages for children and young people with complex health needs.Key Responsibilities: Undertake Children''s Continuing Care assessments and reviews in line with the National Framework and local policies. Assess the healthcare needs of children and young people with complex needs. Produce high quality assessment reports and recommendations. Participate in multidisciplinary team meetings and case discussions. Work collaboratively with families, healthcare professionals, education providers and local authority colleagues. Support care planning and ongoing review processes. Ensure accurate and timely documentation is maintained.Essential Requirements: Registered Nurse with current NMC registration. Previous experience undertaking Children''s Continuing Care (CHC) assessments. Strong understanding of Continuing Care processes and frameworks. Experience working within NHS services. Excellent communication, report writing and stakeholder management skills.
