Service Overview
This role sits within a well-established Community Neuro Service, delivering rehabilitation and management for adults with stroke and other neurological conditions in community settings. The service supports early discharge from hospital and ongoing rehabilitation at home, working across ESD, stroke and neuro pathways.
Role Summary
The postholder will provide a specialist Speech and Language Therapy service to adults with communication and swallowing difficulties arising from neurological conditions. You will work as part of a multidisciplinary team delivering high-quality, patient-centred care across community pathways.
Key Responsibilities
Deliver specialist SLT assessment and intervention for communication and dysphagia post-stroke
Manage and prioritise a complex community caseload independently
Work collaboratively across ESD, stroke, and neuro MDT pathways
Provide advice, education and support to patients, carers, and wider teams
Assess and recommend AAC (alternative and augmentative communication) aids
Support and supervise junior staff and contribute to team development
Deliver training and contribute to service improvement and CPD initiatives
Maintain accurate clinical records in line with RCSLT and Trust standards
Essential Requirements
Degree in Speech and Language Therapy (or equivalent)
HCPC registration
Member of the Royal College of Speech and Language Therapists (RCSLT)
Post-registration experience working with adult neuro / stroke patients
Experience managing dysphagia independently
Experience working within a multidisciplinary team
Strong communication, clinical reasoning and caseload management skills
Desirable Experience
Community-based SLT experience
Experience within Early Supported Discharge (ESD) services
Knowledge of videofluoroscopy assessments
Experience with AAC systems
Experience supervising junior staff or students
Additional Information
Community role - travel across the borough required
Candidates must be confident working autonomously and within MDTs
Compliance required: HCPC, DBS, mandatory training, references
