The role combines screening referrals, conducting assessments, and coordinating care within a multidisciplinary team framework. The nurse will also undertake shift co-ordinator responsibilities for one day per week. Key Responsibilities: Referral Screening & Triage: Identify urgent, crisis, and routine cases; take calls from professionals, parents, or guardians regarding potential mental health concerns. Initial Assessments: Conduct face-to-face assessments to determine the needs of children and young people. Multidisciplinary Team Meetings: Attend daily MDT meetings to discuss outcomes, agree on interventions, and coordinate care plans.
